Measurable Outcomes

At Care Afloat we realise that you, as purchasers, have a right to assess the effectiveness of individual placements with us.
This is notoriously difficult to measure when dealing with developmental and emotional needs of young people.
As we tend to deal with young people with previous multiple placements, we measure our success in the 1st year against a series of measurable and witnessable parameters, both prior to, immediately after and following the first 6 months of placement.

These include:

Number of: Absconsions, Offences, Physical Interventions, Notifiable Events
Appropriate Family Contacts Attendance and participation at Reviews
School Attendance.

Nevertheless, these must be taken in context relating to each young person and are only one part of the picture.

ICHEP

Together with these parameters, we have designed our unique Individual Care, Health and Education Plan (ICHEP). This document takes as its targets the 5 Every Child Matters outcomes of:

  • Be Healthy
  • Stay Safe
  • Enjoy and Achieve
  • Make a Positive Contribution
  • Achieve Economic Wellbeing

This individualised document is drawn up by all proffessionals involved in delivering the young persons holistsic package of Care, Health (CHHT) and education (Learn4Life). The young people are central to drawing up this document and are consulted every month when progress is reviewed. New targets can then be drawn if success is achieved. Social workers are constantly amazed at the rapid and consistent changes in the Young People residing with us.
